How should a reader hold "approved somewhere, not here"?

Carefully, and it is a genuinely good question because the answer is not "approval is approval".

Regulators reach different conclusions on the same dossier, and the reasons vary: different evidentiary thresholds, different comparators, different unmet need in that population, and sometimes simply that nobody filed. An approval in one jurisdiction is real evidence that a competent body looked at the data and was satisfied — it is not evidence that every body would be.

The practical version: an approval elsewhere raises the floor on how seriously to take it, and tells you nothing about whether the vial in front of you was made to the standard that approval required.
